Leptin bị chống lại bởi các hoạt động của hormone ghrelin, "hoocmon đói". Cả hai loại hooc môn đều tác động lên thụ thể trong nhân trung vị của vùng dưới đồi nhằm điều chỉnh sự thèm ăn để đạt được sự cân bằng nội sinh.[3] Trong bệnh béo phì, tương tự như sự đề kháng insulin ở bệnh đái tháo đường týp 2, có sự nhạy cảm với leptin giảm, dẫn tới sự không có khả năng phát hiện được cảm giác no mặc dù các cửa hàng có năng lượng cao. [4]Mặc dù quy định của các chất béo dự trữ được coi là chức năng chính của leptin, nó cũng đóng một vai trò trong các quá trình sinh lý khác, chứng minh bởi các mạng lưới nhiều của nó tổng hợp khác với các tế bào chất béo, và nhiều loại tế bào bên cạnh các tế bào hypothalamic có thụ thể leptin. Nhiều chức năng bổ sung này vẫn chưa được xác định.[5][6]
